The Team & Role
Join our dedicated Access Team, the first point of contact for clients seeking information and referrals to the wide range of community health, rehabilitation and specialist services offered by Bayside Health – Peninsula Care Group. As part of this team, you’ll play a key role in helping clients navigate their care options and connecting them with the most appropriate services based on their needs.
In this role, you’ll manage referrals efficiently, contact clients to confirm details and consent, and conduct initial needs identification to assess eligibility, priority and service suitability. You’ll maintain accurate client data in systems such as iPM and UNITI, provide clear information about available services and alternative pathways, and assist with appointment booking where required. A strong understanding of Peninsula Care Group services, community sector supports and funding eligibility will enable you to guide clients effectively.
Success in this role relies on strong communication, emotional intelligence, problem‑solving and teamwork. You’ll work in a supportive environment that values organisation, adaptability and continuous improvement—all while ensuring clients receive timely, respectful and consumer‑centred service.

Skills & Experience Required
- A relevant tertiary qualification and/or significant relevant experience in the health or community health services sector
- Advanced interviewing and engagement skills
- Proficiency in the Microsoft Office suite, particularly MS Word and other data base software programs
- Demonstrates advanced computer and keyboard skills
- Highly developed verbal and written communication skills*/
