Bayside Health was formed following the merger of Alfred Health, Bass Coast Health, Gippsland Southern Health Service, Kooweerup Regional Health Service and Peninsula Health on 1 January 2026. 

  

The Team & Role 

The newly introduced Single Assessment System (SAS) aims to streamline aged care assessment services with one set of eligibility criteria and one assessment process for clients to improve accessibility, choice and inclusion. The Single Assessment System Program provides both Comprehensive and Home Support Assessments for older people to determine eligibility for a range of Commonwealth programs including Commonwealth Home Support Program (CHSP), delegable Commonwealth-funded services such as Residential Respite, Permanent Care, Home Care Packages and Flexible Care Options.

The service operates Monday to Friday during business hours.

Skills & Experience Required 

  •  Bachelor Degree in Nursing or allied health qualification, registered with APHRA

  • Experienced Aged Care Assessment Clinicians who are credentialed to conduct assessments through My Aged Care, with demonstrated knowledge of the aged care sector, assessment frameworks, and person-centred care principles.
  • Understanding and / or experience working in the acute, rehabilitation or community services for the aged

  •  Demonstrated knowledge of and experience in the provision of Primary Care services especially in respect of assessment and service delivery.

  • Demonstrates both short and longer term care planning for clients

  • Excellent written and verbal communication skills.

  •  Postgraduate studies Community Health or Gerontology

  • Hold a current driver's licence and willing to travel to clients homes within designated catchment
