The Team & Role

This Grade 2 Physiotherapist position is in the Early Intervention in Chronic Disease Team (EIiCD), which is a multidisciplinary team in Community health at Bayside Health Peninsula that works in a holistic manner to help clients achieve their goals, and promotes self-management. The EIiCD physiotherapist provides predominantly centre-based services to clients presenting with musculoskeletal, respiratory, cardiovascular, diabetic and neurological conditions. Assessments and interventions are tailored to the individual to optimise their health and wellbeing. Patient centred care plans are developed and clients are managed via face-to-face interventions, telehealth, phone consults and/or group programs.

Skills & Experience Required

Essential
•    Current registration with AHPRA. 
•    Experience working in an outpatient setting. 
•    Proven ability to work effectively within multidisciplinary teams.  
•    Demonstrated commitment to delivering high quality outcomes for consumers. 
•    Demonstrated clinical knowledge and skills commensurate with experience.
