  • Classification: Registered Nurse (RN GR3B Y1 (ZJ1)) or Social worker - Grade 2 (Year 1 - Year 4)

 

On 1 January 2026 five health services (Kooweerup Regional Health Service, Peninsula Health, Alfred Health, Bass Coast Health and Gippsland Southern Health Service) will come together to create Bayside Health. A newly merged health service with more than 22,000 employees caring for a community of over 1.2 million Victorians.

The new health service will provide people with the best possible care with services that support them throughout their lives. The merger will also strengthen our workforce, providing new training, education and career opportunities. Subject to final approval on 1 January 2026, all employees from the five health services will transfer to Bayside Health.

The Team and Role

Peninsula Health is committed to the implementation of Advance Care Planning (ACP) with a particular focus on adults with chronic complex disease, life-threatening illnesses and the frail aged. ACP facilitates an individual to appoint their medical treatment decision-maker, appointed support person and to document their care preferences in a legitimate format known as an Advance Care Directive or Instructional Directive.

The Advanced Care Planning Clinician works as part of a team who are responsible for providing ACP support across PH and in the community.

Skills and Experience Required

We anticipate you will have:

  • Eligible for registration with the relevant professional organisation (Registered Nurse or Social Work) .
  • Sound understanding of Advance Care Planning aims and underlying principles.
  • Experience in palliative care, end-of-life care, Advance Care Planning, and or Voluntary Assisted Dying.
  • Commitment to work as part of a specialised team to achieve the ACP Program aims.
  • Ability to supervise undergraduate students.
  • Current Victorian Driver’s Licence.
  • Working with Children Check.

Sub-grading will be based on years of experience.
