What You Will Be Doing  
At Home Subacute aims to deliver quality, evidenced based, multidisciplinary subacute care to patients in their own home, which would otherwise have been delivered in a hospital bed. There is a focus on shifting rehabilitation care out of the hospital setting, and into the home environment, to empower people to optimise their recovery, function and quality of life, with the support and guidance of home based rehabilitation experts.
This role, as part of At Home Subacute will deliver care to patients requiring rehabilitation, geriatric evaluation and management and palliative care in the home and community. Care will be delivered flexibly, and include a combination of home visits, clinic visits, group therapy and telehealth consultations.

What You Need 
We anticipate you will have: 
•    Certificate 4 in Allied Health Assistant 
•    Demonstrated experience as a Grade 2 AHA.
•    Current Victorian drivers licence
•    Demonstrated competency to progress Allied Health programs within defined client groups.
•    Capacity to work independently and autonomously in the community. 
•    A sound understanding of gr 3 AHA scope of practice in the rehabilitation and community settings

*Please refer to Health and Allied Services, Managers and Administrative Workers (Victorian Public Sector) (Single Interest Employers) Enterprise Agreement 2021-2025  for salary range. Sub-grading will be based on years of experience.
