•    Classification: Allied Health Assistant IN30 (Grade 3*) plus super

The Team & Role
The Transition Care Program (TCP) at Bayside Health – Peninsula Care Group is seeking a dedicated team member to support the delivery of high-quality, person-centred care within a multidisciplinary environment across the Mornington Peninsula. Working closely with Physiotherapists, Occupational Therapists, Social Workers, Dietitians and Speech Pathologists, this role focuses on providing safe and effective therapy support through both individual and group sessions. Responsibilities include assisting with therapy programs (residential and home-based), supporting client mobility and transfers, monitoring and escalating changes in patient condition, and maintaining accurate documentation. The role also contributes to care planning, continuous service improvement, and adherence to relevant policies, standards and legislation. This is an excellent opportunity to support Allied Health professionals, enhance patient outcomes, and play an active role in a collaborative care team.

Skills & Experience Required

Essential    
•    Certificate 4 in Allied Health Assistant ( AHA)
•    Demonstrated experience as an AHA.
•    Demonstrated competency in the safe prescription of gait aids to defined client group
•    Demonstrated competency to progress exercise programs with defined client groups.
•    Demonstrated competency in the delivery of group therapy programs
•    Ability to work as an effective team member
•    Capacity to work independent and autonomously
•    Ability to plan and manage own workload
•    High standard of written and verbal skills
•    Flexible and creative when dealing with problems
•    Able to function in a reliable and courteous manner
•    Computer literate
•    Ability to meet the physical requirement of the position.  

Desirable    
•     Experience in working within subacute/Aged Care Allied Health settings.
•     Experience in working within Transition Care Program
