The Team & Role
At Home Subacute aims to deliver quality, evidenced based, multidisciplinary subacute care to patients in their own home, which would otherwise have been delivered in a hospital bed. There is a focus on shifting rehabilitation care out of the hospital setting into the home environment to empower people to optimise their recovery, function and quality of life, with the support and guidance of home based rehabilitation experts.
This role, as part of At Home Subacute will deliver care to patients requiring rehabilitation, geriatric evaluation and management and palliative care in the home and community. Care will be delivered flexibly, and include a combination of home visits, clinic visits, group therapy and telehealth consultations.
This grade 3 Allied Health Assistant role will support the multidisciplinary Allied Health team within At Home Subacute, including physiotherapy, occupational therapy, speech pathology, social work, dietetics, psychology. The Allied Health Assistant will provide a wide variety of subacute interventions across the multidisciplinary team, to patients with a variety of conditions.

Skills & Experience Required
- Demonstrated experience as a Grade 2 AHA
- Certificate 4 in Allied Health
- Current drivers licence
- Capacity to work independently and autonomously in the community
- A sound understanding of gr 3 AHA scope of practice in the rehabilitation and community settings
Desirable
- Demonstrated competency to progress Allied Health programs within defined client groups
- Experience in the subacute or community settings
