The Team & Role
This role is part of the Better at Home strategy, a body of work aimed at providing as much care as is safely possible, outside a bed-based environment by broadening the scope of its home-based services, addressing service gaps and enhancing and integrating these services to result in a more seamless response for patients, their families and carers.
Better@Home has three main work streams:
•Journey-to-Home – pathways of care to and from hospital that reduce the reliance on a bed-based service;
•Care@Home – services that provide care outside a bed-based service; and
•Stay@Home – services that help keep people well and remaining at home.
Specifically, this role is part of the Care@Home work stream and will deliver care to ortho, neuro, general reconditioning and palliative patients requiring rehabilitation, geriatric evaluation and management (GEM), and palliative care in the home and community. Care will include a combination of home visits, clinic visits, group therapy and telehealth consultations. The model provides an alternative to hospital care at an intensity greater than what can be provided by an ambulatory model. It is a 24-hour, 7 day a week service and the role will include after hours on-call duties to ensure the ongoing care and safety of patients.
The Associate Nurse Unit Manager (ANUM) is a valued member of the AT Home leadership team, committed to upholding Peninsula Health’s values and meeting organisational goals. Working closely with the Nurse Unit Manager (NUM), the ANUM will be required to maintain bed flow and manage resource allocation within the service. They will coordinate, plan and manage patient admissions to AT Home bed substitution and or diversionary care programs, reduce acute/subacute length of stay and readmissions through comprehensive assessment and collaboration with the multidisciplinary acute care and @Home rehab, GEM and palliative care teams. They will function as an integral member of the multidisciplinary team working competently within their scope of practice to deliver safe, evidence-based person-centred care to patients in their home environment to achieve optimal health outcomes or a dignified death.
The ANUM will also provide leadership and direction to all nursing staff working within the service. They will support the NUM in all aspects of the role and when required, act in their role to provide effective leadership and management. This position will be required to deliver services across the Peninsula Health catchment, and as such, may be required to work flexibly across any of the Peninsula Health sites. To ensure clinical safety and continuity of patient centred care there is a requirement that all ANUM positions work a minimum of 0.6 EFT per week.

Skills & Experience Required
Essential
- Registered Nurse with current AHPRA registration.
- Prepares written documentation using clear, concise and grammatically correct language.
- Clearly and confidently communicates with key contacts and welcomes constructive feedback.
- Demonstrated ability to co-ordinate patient care, set priorities and work independently.
- Demonstrated clinical knowledge and skills commensurate with experience and scope of nursing practice with a particular focus on rehabilitation nursing skills:
- Promotes participation in self-management and health-optimisation strategies.
- Advocates for patients and family/carer decision making, goal setting and decision making, modifying as required.
- Communicates well with and relates to consumers from a wide range of cultures and backgrounds.
- Supports individuals and the team, delegating responsibilities appropriately.
- Demonstrated commitment to delivering high quality outcomes for consumers.
- Understands the issues and pressures to which the organisation has to respond.
- Seeks all relevant information for problem solving.
- Uses a range of software application features for word processing, spread sheets etc.
- Accepts responsibilities for own actions.
- Current Victoria driver’s licence.
Desirable
- Has worked as a Registered Nurse in a Subacute and/or Community Environment.
- Experience in providing patient families/carers with consultative care and education that addresses specific patient needs such as, but not limited to, ADL management, mobility, falls prevention, fatigue management, communication, diet, swallowing, disease management, continence and optimising skin integrity.
- Trained in administration of the Functional Independence Measure (FIM) with current credentialed status.
- Identifies and understands the long term impact of particular courses of action on the organisation’s objectives
- Experience in a leadership or management role
- Working towards or having completed a postgraduate qualification in health management
