What You Will Be Doing

The Older (Aged) Persons Community Mental Health Team (APCMHT) is a specialist multi-disciplinary and integrated service that provides a broad range of interventions, care coordination and support for older people experiencing severe and persistent mental health disorders and their carers.

 

The APCMHT Clinician will plan, evaluate and review care, undertaking face-to-face assessments and contributing to recovery care plans. The role requires frequent liaison with assessments and contributing to recovery care plans. The role requires frequent liaison with external services and internal multi-disciplinary team.

 

Position Description

 

What You Need

We anticipate you will have:

• Registration as a Registered Nurse, Social Worker, Occupational Therapist or Clinical Psychologist with relevant registration RPN ONLY: Post graduate qualification in Mental Health or specialist undergraduate Mental Health program.

• Proven ability in the management of clients with a major mental health illness and in the techniques of assessment and crisis intervention, and recovery frameworks.

• Experience working in an acute inpatient.

• Current Victorian Drivers Licence.
