What You Will Be Doing
The Transition Care Program (TCP) at Peninsula Health sits within the Rehabilitation, Ageing, Pain and Palliative Care Services (RAPPS) cluster and provides a comprehensive range of multidisciplinary services across multiple service sites throughout the Mornington Peninsula.
TCP works with older people at the interface between hospital, the community and home aiming to minimise inappropriate extended hospital. This position is full time limited term until June 2026.

What You Need
We anticipate you will have:
- Eligibilty for Australian Association of Social Work Membership
- Current and valid Driver’s Licence
- Proficient in using Microsoft office Suite and computer programs
- Experience in a sub-acute and/or community setting (desirable)
- Experience as a Case Manager with sound understanding of aged care (desirable)

*Please refer to (Allied Health Professionals (Victorian Public Sector) (Single Interest Employers) Enterprise Agreement 2021-2026 salary circular for salary range. Sub-grading will be based on years of experience.
