The Team & Role 

Drive stronger system connections between acute, subacute, primary and aged sectors, ensuring older Victorians receive the right care in the right place.  The Residential in-Reach Pilot aims to provide short-term, person-centred multidisciplinary support to older residents of Residential Aged Care Facilities (RACFs) following hospitalisation. The program enhances recovery, reduces avoidable hospital readmissions, and improves quality of life through integrated, collaborative, and timely care. We have a fixed term Full time / Part time role available until 30 June 2028.

The Clinical Nurse Consultant (CNC) provides advanced clinical leadership and ensures high-quality, evidence-based nursing care and coordination for older residents transitioning back to RACFs after hospitalisation. The role includes clinical leadership, direct care, education, consultation, and contribution to service development and evaluation, consistent with the aims of the Strengthening Medicare – Support for Older Australians initiative

Skills & Experience Required 

• Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA).
• Postgraduate qualifications in gerontology, aged care, or related specialty (or working toward completion).
• Extensive experience in aged care, subacute, or community health settings.
• Demonstrated advanced clinical assessment, decision-making, and care coordination skills.
• Sound understanding of the health and aged care interface, hospital avoidance, and post-acute models of care.
• Experience in service development, pilot program implementation, or quality improvement.
• Experience delivering education to health or aged care professionals.
• Current Victorian driver's license, police check and aged care check
