On 1st January 2026, five health services (Kooweerup Regional Health Service, Peninsula Health, Alfred Health, Bass Coast Health and Gippsland Southern Health Service) will come together to create Bayside Health; a newly merged health service with more than 22,000 employees caring for a community of over 1.2 million Victorians.
The new health service will provide people with the best possible care with services that support them throughout their lives. The merger will also strengthen our workforce, providing new training, education and career opportunities. Subject to final approval on 1st January 2026, all employees from the five health services will transfer to Bayside Health.
The Team & Role
The Community Care medical team is one of diverse generalist skills set whilst utilising other specialist skills (such as Geriatrics, Palliative Medicine, Emergency Medicine, General Practice and General Medicine).
This medical position within Community Care has a broad range of roles including response to calls from Residential Aged and Supported Care Facilities as well as providing community based medical interventions for referrals from Peninsula Health clinicians, Ambulance Victoria, Victorian Virtual Emergency Department (VVED), General Practitioners and other community health professionals and perform consultations via the Health Direct telehealth platform.
In addition, Community Care Doctors provide oversight and care to Hospital in the Home patients, non-admitted Community Care clients and Virtual Monitoring Team clients. This position will respond to the demand in the Emergency Department and Acute Inpatient wards by facilitating patient discharges as well as reducing hospital attendance and admission by providing clinically appropriate supported care in the community. It also involves supporting service partners in the newly forming Bayside Health through ward service and support at Koo Wee Rup Hospital.
It also provides ongoing patient care at Carinya Aged Persons Mental Health Residential Care Facility and also supports medical care through the Peninsula Opioid Pharmacotherapy Initiative (POPI) Clinic.
Community Care Doctors are supported in providing care to patients by HITH and community clinical nurses and allied health clinicians.

Skills and Experience Required
Essential
- Registered Medical Practitioner with current registration with Australian Health Practitioner Regulation Agency (APHRA)
- Fellowship of the Australasian College of Emergency Medicine, or the Royal Australian College of General Practitioners, or the Royal Australian College of Physicians or equivalent
- Current Victorian Drivers Licence
- Previous experience in working within a clinical area relevant to the requirements of the community physician role
Desirable
- Medication Assisted Treatment for Opioid Dependence (MATOD) RACGP Training
- Mental Health Skills Training recognised by RACGP, ACRRM and Medicare
- Experience in research and quality improvement activities in service development.
- Knowledge of the local demographics of the region and the impact upon the planning and delivery of services.
- Flexible with ability to explore alternative models of service provision
