The Team & Role
The Community Health Independence Brokerage Coordinator is a vital role within Bayside Health’s Community Care Department, supporting strong connections between internal teams and external service providers. The position focuses on coordinating communication, maintaining accurate data systems, and ensuring services delivered align with contractual and billing requirements.
The role is responsible for managing and reporting on program performance data, supporting quality improvement activities, and using insights to inform integrated care planning. It also provides system oversight, staff training, and administrative coordination to ensure brokerage processes run efficiently and support high‑quality, consumer‑focused care.

Skills & Experience Required
- An understanding of clinical service delivery models and/or brokerage frameworks, particularly in relation to coordinating services with external providers, managing billing processes, and maintaining accurate service data.
- Broad administrative experience and up-to-date skills and knowledge in relevant IT-related data management systems.
- Experience and knowledge regarding public health services systems and processes.
- Commitment to a collaborative and team oriented approach to business practices.
- Experience in working both independently and as a part of a multi- disciplinary team, with the ability to work effectively, supportively and flexibly.
- Willingness to maintain and advance knowledge relevant to position.
