On 1 January 2026 five health services (Kooweerup Regional Health Service, Peninsula Health, Alfred Health, Bass Coast Health and Gippsland Southern Health Service) will come together to create Bayside Health. A newly merged health service with more than 22,000 employees caring for a community of over 1.2 million Victorians.

The new health service will provide people with the best possible care with services that support them throughout their lives. The merger will also strengthen our workforce, providing new training, education and career opportunities. Subject to final approval on 1 January 2026, all employees from the five health services will transfer to Bayside Health.

What You Will Be Doing  
The At Home Subacute program is designed to provide high quality, evidence-based, multidisciplinary rehabilitation care to patients in the comfort of their own homes, replicating services that would typically be delivered in a hospital setting. 
The Grade 2 Dietitian will work collaboratively within the multidisciplinary team to ensure optimal outcomes for clients in the community. A strong focus will be placed on client-centred goal setting, timely care delivery, and planned discharge, with a collaborative team approach. The role will involve applying clinical best practices to a wide range of conditions, including musculoskeletal, neurological, geriatric, palliative care, HBCC and HITH patients. The Dietitian will be required to deliver services across the entire Peninsula Health catchment and may be expected to work flexibly across multiple Peninsula Health sites.

What You Need 
We anticipate you will have:

  • Current Accredited Practising Dietitian
  • Member of the Dietitians Association of Australia
  • Ability to competently perform high quality Dietetic assessment, treatment and patient 
    management, including thorough and appropriate discharge planning
  •  Demonstrated experience and competence in rehabilitation across a wide variety of streams
  •  Current Victorian Driver’s licence
  • Demonstrated experience in the supervision of Dietetic staff and students (desirable)
  • Relevant post-graduate qualification or working towards (desirable)
