What You Will Be Doing  
The Early Intervention in Chronic Disease (EIiCD) program plays an important role in supporting people with chronic conditions through HACC, CHSP and ICDM funding. The EIiCD program delivers person-centred multidisciplinary care across all Peninsula Health Community Health sites. Service provision should be provided in a manner that is well planned, integrated, and supports the person’s capacity to self-manage  
The Northern Team Leader works within the EIiCD program leadership team to facilitate on-going quality improvement in service delivery to clients, and to operationally support the staff within the Northern EIiCD team. The Northern Team Leader holds both a leadership role and a clinical caseload. This position is a parental leave position, limited term for 9 months.

What You Need 
We anticipate you will have: 
Essential
•    Current registration with relevant Accredited National Agency such as AHPRA/ESSA/Dietitians Australia or APD.
•    Experience working in a community or outpatient setting.  
•    Understanding of current EIiCD funding sources and provider requirements for HACC, CHSP, and ICDM. 
•    A current Victorian driver’s licence. 
Desirable:
•    An understanding of the concepts and strategies of population health promotion and the chronic care model. 
•    Qualifications or experience in adult education and group training. 
•    Experience in operational and/ or clinical leadership, staff and student supervision and  mentoring.
