The Disability Services Team supports people with disabilities across inpatient and community settings, improving access, inclusion, and health outcomes. The team works collaboratively to navigate complex disability pathways and embed inclusive practice across the health service.
As the Disability Services Team OT Clinical Lead, you will provide specialist occupational therapy expertise and act as the primary point of contact for disability‑related clinical support. The role focuses on NDIS navigation, comprehensive disability assessments, transdisciplinary consultation, and advocacy for people with complex needs.
You will work closely with multidisciplinary teams to support complex care planning, reduce preventable hospital re‑presentations, and drive system‑wide improvements aligned with the Inclusive Victoria State Disability Plan and the Peninsula Health Disability Action Plan, helping build a disability‑confident and inclusive health service.
Key Responsibilities:
- Deliver advanced occupational therapy expertise and evidence-based care for people with disabilities.
- Implement key goals of care from the Victoria: State Disability Plan 2022-2026 and the Peninsula Health Disability Action Plan 2023-2025.
- NDIS capacity building across the Peninsula Care Group network
- Conduct Comprehensive Disability Needs Assessment.
- Complete OT functional assessments and complete NDIA functional capacity reports in collaboration with the inpatient treating team to support complex NDIS pathways.
- Collaborate with community and NDIS service providers
- Support teams in managing complex disability care planning and provide preventative care for PWD at risk of re-presenting to hospital.

Skills & Experience Required
- A relevant graduate qualification in Allied Health and current APHRA registration or professional association membership for non-AHPRA professions.
- High level understanding and experience of working with PWD and the NDIS and health interface
- Skills and experience navigating complex systems for people with a disability who present with a broad range of needs
- Established skills in completing holistic assessments with a functional impact focus
- Highly-developed communication, report writing and interpersonal skills
- Experience with change management and stakeholder engagement, including a demonstrated ability to communicate and collaborate effectively with key stakeholders both within and external to an organisation.
- Commitment to ongoing professional development and continuous improvement
- Demonstrated competence in Microsoft Word, Excel, Outlook, Teams and the internet
- Demonstrated experience in project delivery including evaluation processes and the ability to maintain accurate records for reporting and future planning purposes.
- Eligibility for registration with AHPRA or other regulatory bodies. Compliance documents: tertiary qualification, Working with Children Check, National Police Check, and Victorian Driver's license
