What You Will Be Doing
The Peninsula Health Care Finder program supports vulnerable clients (over the age of 50) who are at risk of homelessness with housing support and service system navigation.
The Case Manager role provides specialist and intensive case management to help clients understand and access housing options and aged care services, and connect with other relevant supports in their community. The program provides case management via assertive outreach to assist older people who face barriers to accessing housing and services and are vulnerable, marginalised, and disadvantaged. This Case manager is required to provide holistic support and pathways to address their client’s wellbeing and health needs.

What You Need
We anticipate you will have:
• A qualification in Social Work, Occupational Therapy, or Community Development Work
• Holds membership with relevant professional association or AHPRA registration
• Experience in working with clients with complex needs.
• Experience in case management or similar modalities.
• Experience working in health care, community health or outreach settings.
• An applied understanding of the issues and implications of homelessness for clients with complex health and social needs.
• Demonstrated commitment to the Social Model of Health and principles of trauma informed practice.
• Current driver’s licence and willingness to transport clients where appropriate.

*Please refer to Allied Health Professionals (Victorian Public Sector) (Single Interest Employers) Enterprise Agreement 2021-2026 for salary range. Sub-grading will be based on years of experience.
