Bayside Health was formed following the merger of Alfred Health, Bass Coast Health, Gippsland Southern Health Service, Kooweerup Regional Health Service and Peninsula Health on 1 January 2026. 


The Team & Role
 

The Intake Coordinator role is part of the Better At Home strategy, the team provides as much care as is safely possible, outside a bed-based environment by broadening the scope of its home-based services, addressing service gaps and enhancing and integrating these services to result in a more seamless response for patients, their families and carers. It is a 24-hour, 7 day a week service.

The Intake Nurse Coordinator (INC) is essential in the continuum of care for individuals with disability, chronic illness and/or recovering from surgery or acute illness. Working closely with the NUM and ANUM, the INC is a key driver in facilitating patient flow from acute, subacute or community care settings in to the relevant At Home subacute rehab, GEM or palliative care program whilst promoting timely and optimal patient outcomes. The INC will coordinate, plan and manage patient admissions to At Home subacute bed substitution or diversionary care programs, reduce acute/subacute length of stay and readmissions through comprehensive assessment and collaboration with the multidisciplinary acute care and At Home subacute, rehab, GEM and palliative care teams.

Skills & Experience Required
 

  • Registered Nurse with current AHPRA registration
  • Current Full Victorian Drivers Licence
  • Demonstrated commitment to delivering high quality outcomes for patients
  • Understands the issues and pressures to which the organisation has to respond
  • Is able to identify continuous improvement opportunities for improved patient/consumer outcomes
