What You Will Be Doing
The Older (Aged) Persons Community Mental Health Program (APCMHP) is a specialist multi-disciplinary and integrated service that provides care coordination through a community based assertive outreach model for older people experiencing severe and persistent mental health disorders or dementia with significant associated behaviours, where the persons or their carers may benefit from specialist psychiatric consultation.
The team provides a broad range of treatment, case management, care interventions and support for consumers living in the Peninsula Health Mental Health and Wellbeing Service catchment. There are three distinct streams within the Older (Aged) Persons Community Mental Health Team (APCMHT) which includes the Aged Persons Intensive Community Assessment and Treatment Team (APICATT), Case Management Team and the Residential Support Program (RSP).
The Older (Aged) Persons Mental Health Team Nurse Practitioner candidate (APCMHT NPC) is a Registered Nurse who is acquiring the expert knowledge base, complex decision-making skills and clinical competencies in order to work towards becoming endorsed by the Australian Health Practitioners Regulatory Authority (AHPRA) as a Nurse Practitioner.
What You Need
We anticipate you will have:
• Registered Nurse with Mental Health Nursing qualifications such as a Post Graduate Diploma in Psychiatric/Mental Health Nursing or a Specialist Undergraduate Psychiatric/Mental Health Nursing Program with a current registration with AHPRA
• Eligible to undertake a Nurse Practitioner candidacy as recognised by the Nursing and Midwifery Board of Australia via AHPRA (with no conditions of registration relating to unsatisfactory professional performance or unprofessional conduct)
• Enrolled in, or willing to enrol as soon as possible, in a Nursing and Midwifery Board of Australia (NMBA) approved program of study that will lead to endorsement as Mental Health Nurse Practitioner
• Extensive experience (minimum 5 years) in a Senior Mental Health nursing role in a range of service settings particularly in older person’s mental health care
