What you Will Be Doing
The Transition Care Program (TCP) at Peninsula Health sits within the Rehabilitation, Ageing, Pain and Palliative Care Services (RAPPS) cluster and provides a comprehensive range of multidisciplinary services across multiple service sites across the Mornington Peninsula. TCP works with older people at the interface between hospital, the community and home and aims to minimise inappropriate extended hospital lengths of stay and prevent unnecessary admissions to the Emergency Department.

What You Need
We anticipate you will have:
• Bachelor of Occupational Therapy, Masters of Occupational Therapy Practice or Bachelor of Applied Science (Occupational Therapy) or equivalent
•Current Registration with the Australian Health Practitioner Regulation Agency (AHPRA) – Occupational Therapy, with no restrictions.
• A minimum of 3 years professional experience in working with older adults
• Current Driver’s Licence
• Capacity to supervise OT students, Grade 1 Occupational Therapists and Allied Health Assistants
• Computer literate

*Please refer to Allied Health Professionals (Victorian Public Sector) (Single Interest Employers) Enterprise Agreement 2021-2026 for salary range. Sub-grading will be based on years of experience.
