Bayside Health was formed following the merger of Alfred Health, Bass Coast Health, Gippsland Southern Health Service, Kooweerup Regional Health Service and Peninsula Health on 1 January 2026.
The Team & Role
The Transition Care Program (TCP) sits within the Rehabilitation, Ageing, Pain and Palliative Care Services (RAPPS) cluster at Peninsula Health and provides multidisciplinary care to older people across the Mornington Peninsula. TCP supports consumers transitioning from hospital to home or community, with a focus on maintaining independence, reducing length of stay and preventing avoidable hospital readmissions.
As a Grade 2 Occupational Therapist, you will manage a varied caseload within a primarily community‑based role, delivering high‑quality assessment and intervention across a range of aged‑care presentations including orthopaedics, neurology, dementia and falls. The role offers the opportunity to work across multiple sites on the Peninsula (including Frankston and Rosebud), with flexibility to work from home where appropriate.
Working as part of a supportive multidisciplinary team, you will contribute to excellent consumer outcomes, support junior staff and students, and actively participate in service improvement and continuous learning.

Skills & Experience Required
- Bachelor of Occupational Therapy, Masters of Occupational Therapy Practice or Bachelor of Applied Science (Occupational Therapy) or equivalent.
- Current general registration with the Australian Health Practitioner Regulation Agency (AHPRA) – Occupational Therapy, with no restrictions.
- A minimum of 3 years professional experience in working with older adults.
- Current Driver’s Licence.
- Demonstrated participation in Professional Development.
- Interest and/or experience in Quality improvement and research activities.
- Capacity to supervise OT students, Grade 1 Occupational Therapists and Allied Health Assistants.
- Commitment to Occupational Therapy practice in an appropriate and professional manner.
- Ability to operate computer systems relevant for your role.
