• Salary range - $70,564 per annum fulltime to $103,724 per annum fulltime (dependent on experience).

What You Will Be Doing
The Allied Health Outreach Occupational Therapist works with eligible consumers under the Commonwealth Home Support Program (CHSP) and Home and Community Care Program for Younger People (HACC), providing comprehensive assessment and goal directed care planning to maximise independence and safety in the home and community. The Allied Health Outreach Occupational Therapist is involved in specialist recommendations regarding assistive equipment and home modifications; offering holistic support and education to clients and their carers; the delivery of episodic care and management of referrals to appropriate services and programs

What You Need
We anticipate you will have:
Current registration with the Occupational Therapy Board of Australia (via AHPRA).
Sound knowledge of current Occupational Therapy theory and practice.
Understanding of client self-management principles, the Commonwealth Reablement Model
and Victorian Active Service Model.
A strong interest in working with clients in their home and/or community environment.
Experience and understanding of working as part of a multi-disciplinary team.
Excellent verbal and written communication and report writing skills.
