Bayside Health was formed following the merger of Alfred Health, Bass Coast Health, Gippsland Southern Health Service, Kooweerup Regional Health Service and Peninsula Health on 1 January 2026. 

The Team & Role

The Transition Care Program provides multidisciplinary services to older people across the Mornington Peninsula, supporting individuals as they transition from hospital to home and helping to reduce unnecessary hospital stays and Emergency Department presentations. The program delivers nursing care, personal care, case management and allied health services, supporting 85 community and home-based transition care beds. As a Grade 2 Occupational Therapist, you will manage a diverse aged care caseload, provide high-quality assessment and intervention, work collaboratively within a multidisciplinary team, support junior staff and students, and contribute to service improvement and positive patient outcomes through person-centred care.

Skills & Experience Required

• Bachelor of Occupational Therapy, Masters of Occupational Therapy Practice or Bachelor of Applied Science (Occupational Therapy) or equivalent.
• Current general registration with the Australian Health Practitioner Regulation Agency (AHPRA) – Occupational Therapy, with no restrictions 
• A minimum of 3 years professional experience in working with older adults.  
• Interest and/or experience in Quality improvement and research activities.  
• Capacity to supervise OT students, Grade 1 Occupational Therapists and Allied Health Assistants. 
• Commitment to Occupational Therapy practice in an appropriate and professional manner.
