Bayside Health was formed following the merger of Alfred Health, Bass Coast Health, Gippsland Southern Health Service, Kooweerup Regional Health Service and Peninsula Health on 1 January 2026.
The Team & Role
The Early Intervention in Chronic Disease (EIiCD) team at Peninsula Health delivers group-based physical activity programs that support people living with, or at risk of, chronic disease. These programs aim to improve health, function and wellbeing through safe, evidence-based exercise.
The Physical Activity Group Liaison Worker provides administrative and coordination support to the Peninsula Health Agestrong Exercise Program and other EIiCD physical activity groups, including Healthy Hearts, Pulmonary Rehabilitation, Cancer Rehabilitation and Lower Limb programs. The role acts as a key point of contact for referrals, participant communication and program coordination, working closely with clinicians and group leaders.
This position also builds and maintains relationships with external exercise providers across the Frankston City Council and Mornington Peninsula Shire areas, ensuring Peninsula Health staff have up‑to‑date information and resources to support appropriate community-based referrals.

Skills & Experience Required
- An ability to work autonomously.
- An ability to prepare written documentation using clear, concise and grammatically correct language.
- Ability to clearly and confidently communicate with key contacts, group leaders and participants.
- Establishes and maintains good working relationships with relevant internal and external stake holders.
- Ability to use a range of computer software application features for word processing, spread sheets etc.
- Excellent time management skills.
- Drivers License.
- Relevant qualification for Community Development worker classification.
