The Team & Role

The Forensic Mental Health in Community Health (FMHiCH) Program delivers safe, collaborative, and high-quality mental health services within the community. It also facilitates referrals to other health services to improve both mental and physical health outcomes for clients.  Clients considered eligible to engage in the program need to be aged 18+ and are referred from Community Corrections Service (CCS).  Participants in the program must be on a parole or a Community Correction Order and have a primary diagnosis of a moderate mental illness or disorder and a court-mandated condition requiring Mental Health Treatment and Rehabilitation.  The further need to have been assessed via corrections of being at moderate to high risk of re-offending.
The program aims to proactively engage eligible individuals and provide forensic mental health care tailored to their needs. It adopts a holistic approach by identifying additional health concerns such as chronic illnesses, dental issues, and social needs including housing, employment, and family support. Appropriate referrals are made to community health services and other providers.
Collaboration with CCS case managers is essential for effective care planning and coordination of social support services. This integrated approach ensures comprehensive care and supports successful outcomes for clients.
