Bayside Health was formed following the merger of Alfred Health, Bass Coast Health, Gippsland Southern Health Service, Kooweerup Regional Health Service and Peninsula Health on 1 January 2026.

The Team & Role

This role is part of the At Home Subacute Program, a body of work aimed at providing as much care as is safely possible, outside a bed-based environment by broadening the scope of its home-based services, addressing service gaps and enhancing and integrating these services to result in a more seamless response for patients, their families and carers.

At Home Sub Acute Program has three main work streams:

  • Journey-to-Home – pathways of care to and from hospital that reduce the reliance on a bed- based service;
  • Care At Home – services that provide care outside a bed-based service; and
  • Stay At Home – services that help keep people well and remaining at home.

Specifically, this role is part of the Care At Home Subacute work stream and will deliver care to ortho, neuro, general reconditioning and palliative patients requiring rehabilitation, geriatric evaluation and management (GEM), and palliative care in the home and community. Care will include a combination of home visits, clinic visits, group therapy and telehealth consultations. The model provides an alternative to hospital care at an intensity greater than what can be provided by an ambulatory model. It is a 24-hour, 7 day a week service and the role may include after hours on-call duties to ensure the ongoing care and safety of patients.

The At Home subacute Registered Nurse (RN) is essential in the continuum of care for individuals with disability, chronic illness and/or recovering from surgery or acute illness. They will be required to work in the community providing care to our rehabilitation, GEM and palliative care patients at home. They are valued and integral members of the Peninsula Health multidisciplinary team demonstrating flexibility in caring for patients and their families across the organisation. The RN will work as a sole practitioner when delivering care in our bed substitution or diversionary care programs provided outside the hospital setting. Working as part of a multidisciplinary team, the RN co-ordinates and provides an expert clinical service, in line with relevant standards, guidelines and policies of Peninsula Health.

The RN will work under the direction of the Nurse Unit Manager (NUM) and Associate Nurse Unit Manage (ANUM), working competently within their scope of practice to deliver safe, evidence based person centred care to achieve optimal health outcomes or a dignified death. This position will be required to deliver services across the Peninsula Health catchment, and as such, may be required to work flexibly across any of the Peninsula Health sites.

Skills & Experience Required 

Essential 

  • Registered Nurse with current AHPRA registration.
  • Completion of a graduate year program.
  • Demonstrated commitment to delivering high quality outcomes for consumers.
  • Demonstrated clinical knowledge and skills commensurate with experience and scope of nursing practice with a particular focus on rehabilitative nursing skills:
    • Promotes participation in self-management and health-optimisation strategies.
    • Advocates for patients and family/carer decision making, goal setting and decision making, modifying as required.
    • Provides education to patients and family/carers for ongoing therapies and treatments at home and informed decision making.
  • Demonstrated ability to co-ordinate patient care, set priorities and work independently.
  • Willingness to participate in ongoing clinical rehabilitation education.
  • Ability to work as part of a multi-disciplinary team.
  • Able to use basic computer software applications.
  • Prepares written documentation using clear, concise and grammatically correct language.
  • Communicates well with and relates to consumers from a wide range of cultures and backgrounds.
  • Able to work across a 7 day rotating roster.
  • Current Victorian driver’s licence.

Desirable

  • Trained in administration of the Functional Independence Measure (FIM) with current credentialed status.
  • Demonstrates an understanding of common assistive devices/aids that improve functional performance, self-management and quality of life for patients and their practical/therapeutic application, evaluating effectiveness e.g. gait aids, TENS, prostheses, orthoses, etc.
  • Experience in providing patient families/carers with education that addresses specific patient needs such as, but not limited to, ADL management, mobility, falls prevention, fatigue management, communication, diet, swallowing, disease management, continence and optimising skin integrity.
  • Rehabilitation and/or GEM nursing experience in one or all of the 3 streams of rehabilitation care; orthopaedic, neurological and reconditioning.
  • Able to demonstrate ongoing commitment to professional development.
  • Takes responsibility for own actions and is proactive and self-starting.
  • Is able to identify continuous improvement opportunities for improved patient/consumer outcomes.
