The Team & Role 

The role of the Cardiac Nurse within the Early Intervention in Chronic Disease (EIiCD) team is to provide assessment, intervention, health promotion and education (including self-management strategies) to clients to meet their wellbeing needs. Assessments and interventions are tailored to the individual to optimise their wellbeing and self-management. Client appointments are delivered in the outpatient setting, and can be in person, via telehealth or phone consult. 

The EIiCD Cardiac nurse provides assessment for and coordination of the EIiCD Healthy Heart Group exercise and education program, and collaborates with the EIiCD Physiotherapists, Exercise Physiologists, Dieticians and Allied Health Assistants for the holistic care of clients and for the organisation and running of the Healthy Heart groups.

This position will be based primarily at Frankston Integrated Health Centre but may be required to work across all sites. 

Key Responsibilities

The Cardiac Nurse will contribute to:

  •  Deliver high quality, evidence based cardiac nurse services from the EIiCD team, in Community Health, to a broad range of clients.
  • Work as part of a multi-disciplinary team and be able to work with clients both individually and in group settings.
  • Perform client centred assessments and care plans and provide a flexible treatment approach within the community environment, with a strong emphasis on self-management and outcome evaluation.
  • Provide a key role in initiating, facilitating and providing ongoing quality improvement to the service.
  • Incorporate opportunities for innovative models of cardiac service provision, such as using available information technology, to promote coaching or engaging with specific client cohorts.
  • Develop and maintain collaborative partnerships with key stakeholders in acute inpatient cardiac services and also with key community stakeholders, such as other community health services and clients general practitioners.
  • Provide clinical supervision to nursing students as required by the health service.

Skills & Experience Required

  • Registered Nurse with current AHPRA Registration
  • Additional post graduate training in clinical cardiac condition management.
  • Demonstrated experience and skills in cardiac nursing.
  • Excellent written and verbal communication skills.
  • Experience as a member of a multi-disciplinary team.
  • Ability to supervise students and relevant health professionals, including volunteers.
  • Current drivers licence.

Desirable        

  • Member of the Australian Cardiovascular Health and Rehabilitation Association.
  • Experience in Cardiac Rehabilitation.
  • Experience working in chronic disease management.
  • Experience working in a community health setting.
  • Experience in health coaching and motivational interviewing.
  • Experience in facilitating client group programs.
