What You Will Be Doing
The Registered Nurse will be required to work in Hospital in the Home (HITH). The Registered Nurse (RN) is a valued and integral member of the Peninsula Health multidisciplinary team demonstrating flexibility in caring for patients and their families across the organisation. The RN works competently within their scope of practice to deliver safe, evidence based person centred care
Hospital in the Home is a seven days a week service that maintains the continuum of acute inpatient care. This care can be provided in the patient’s home, residential care facility or other community setting. The Registered Nurse will work as a sole practitioner when delivering care outside the hospital setting. Working as part of a multidisciplinary team, the HITH nurse co-ordinates and provides an expert clinical service at an advanced level, in line with relevant standards, guidelines and policies of Peninsula Health.
What You Need
• Registered Nurse with current AHPRA registration
• HITH or community experience
• Extensive nursing experience with a demonstrated history practicing at a proficient level for comprehensive patient assessment, and recognising, responding and escalating clinical deterioration.
• Demonstrated commitment to ongoing personal and professional development, as evidenced in a Continuing Professional Development (CPD) record/Professional Practice Portfolio.
• IV cannulation or working towards
• A current Victorian driver’s licence

(Single Interest Employers) Enterprise Agreement 2024-2028 for salary range. Sub-grading will be based on years of experience.
