On 1st January 2026, five health services (Kooweerup Regional Health Service, Peninsula Health, Alfred Health, Bass Coast Health and Gippsland Southern Health Service) will come together to create Bayside Health; a newly merged health service with more than 22,000 employees caring for a community of over 1.2 million Victorians.   

  

The new health service will provide people with the best possible care with services that support them throughout their lives. The merger will also strengthen our workforce, providing new training, education and career opportunities. Subject to final approval on 1st January 2026, all employees from the five health services will transfer to Bayside Health. 

 

The Team & Role 

Seniors Lifestyle Victoria, funded by the Department of Health, is a multidisciplinary program that supports older adults in self-management and improved health outcomes. It connects individuals with services across Peninsula Health and the broader community, using health coaching to deliver targeted therapies and foster community connections.

The Coordinator works with each client to create a personalised Action Plan based on their goals. They provide professional interventions as required and serve as the primary point of contact for the client throughout an estimated six-month period. This includes delivering and coordinating services across Peninsula Health and the community, with outreach in homes and community settings.

The role involves integrated care planning, motivational interviewing, and data collection to ensure safe, evidence-based practices and meet key deliverables for the pilot program. The Coordinator also contributes to establishing this innovative program while operating within professional standards and enterprise agreements.

Working as part of a multidisciplinary team, the Coordinator provides expert clinical services aligned with relevant standards, guidelines, and Peninsula Health policies

Skills and Experience Required 

 

 

  • Eligible or Member of relevant professional body
  • Current Victorian Driver’s licence
  • Proven ability to work effectively within multidisciplinary teams
  • Demonstrated commitment to delivering high quality outcomes for consumers
  • Demonstrated clinical knowledge and skills commensurate with experience
  • Motivational Interviewing or Health Coaching experience and understanding
