Job Description: 

•    Permanent Part time (48 hours a fortnight)
•    Located in Frankston
•    Classification: Social Worker (Grade 2 Year 1-4*) plus super

On 1 January 2026 five health services (Kooweerup Regional Health Service, Peninsula Health, Alfred Health, Bass Coast Health and Gippsland Southern Health Service) will come together to create Bayside Health. A newly merged health service with more than 22,000 employees caring for a community of over 1.2 million Victorians.  
 
The new health service will provide people with the best possible care with services that support them throughout their lives. The merger will also strengthen our workforce, providing new training, education and career opportunities. Subject to final approval on 1 January 2026, all employees from the five health services will transfer to Bayside Health.

The Team & Role

At Home Subacute has a focus on shifting rehabilitation care out of the hospital setting, and into the home environment, to empower people to optimise their recovery, function and quality of life, with the support and guidance of home based rehabilitation experts. 

This Grade 2 Social Work role will provide comprehensive psychosocial assessment, intervention and case management services to patients within At Home Subacute and HITH programs, to achieve optimal outcomes for clients in the community setting. There is a strong emphasis on evidence-based practice, client focused goal setting with clear timelines and a collaborative team discharge.

Skills and Experience Required

  • Member or eligibility of the Australian Association of Social Work.
  • Current Victorian Drivers license.
  • Highly developed interpersonal and communication skills.
  • Demonstrated ability and experience to work within a multidisciplinary team.
  • Ability to independently deliver high quality social work services, including comprehensive assessment, counselling, patient management, and thorough discharge planning
  • Previous Social Work experience in a public health setting and good working knowledge of community support services.
  • Demonstrated awareness about home based services.
  • Demonstrated knowledge and application of Social Work theory.
  • Ability to manage a demanding workload and effectively prioritise tasks
  • Demonstrated ability to supervise staff /or students.

*Please refer to Allied Health Professionals (Victorian Public Sector) (Single Interest Employers) Enterprise Agreement 2021-2026  salary circular for salary range. Sub-grading will be based on years of experience.
