Bayside Health was formed following the merger of Alfred Health, Bass Coast Health, Gippsland Southern Health Service, Kooweerup Regional Health Service and Peninsula Health on 1 January 2026.
The Team & Role
The Residential in-Reach Pilot aims to provide short-term, person-centred multidisciplinary support to older residents of Residential Aged Care Facilities (RACFs) following hospitalisation. The program enhances recovery, reduces avoidable hospital readmissions, and improves quality of life through integrated, collaborative, and timely care.
The program aims to strengthen system connections between acute, subacute, primary, and aged care sectors, ensuring older Victorians receive the right care in the right place.
The Social Worker provides specialist psychosocial assessment and care coordination within the RESI in REACH Pilot. The role supports older residents, families, and aged care staff during the post-hospitalisation period, promoting wellbeing, adjustment, and sustainable care arrangements.
The role includes clinical leadership, direct care, education, consultation, and contribution to service development and evaluation, consistent with the aims of the Strengthening Medicare – Support for Older Australians initiative.

Skills & Experience Required
• Bachelor of Social Work (or equivalent) and eligibility for membership with the Australian Association of Social Workers (AASW).
• Demonstrated experience in hospital, aged care, or community health settings.
• Understanding of relevant frameworks including the Aged Care Quality Standards, Strengthening Medicare initiatives, and advance care planning principles.
• Sound understanding of the health and aged care interface, hospital avoidance, and post-acute models of care.
• Experience in service development, pilot program implementation, or quality improvement. Experience delivering education to health or aged care professionals.
• Experience delivering education to health or aged care professionals.
• Current Victorian driver's license, police check and aged care check
