The Team & Role

As a Grade 2 Social Worker at Peninsula University Hospital, you are responsible for delivering high quality assessment and interventions to patients and families.  This role requires an ability to complete comprehensive psychosocial assessments, risk assessments and provide support whilst working collaboratively with multi-disciplinary team members and external services, with a particular focus on proactive discharge planning.  You will also provide clinical consultation, guidance and supervision to Grade 1 Social Workers and Social Work students.

Skills & Experience Required

 

  • Bachelor Degree or Qualifying Masters in Social Work & be eligible for AASW membership.
  • Two or more years post-qualifying professional experience in a hospital or related setting. 
  • Proven ability to work effectively within multidisciplinary teams.
  • Demonstrated ability to supervise staff and/or students.
  • Demonstrated excellence in comprehensive assessment, discharge planning and counselling.
  • Advanced demonstrated knowledge and application of Social Work theory.
  • Demonstrated skills and good working knowledge of community support services.
  • Established computer literacy skills.
  • Current Drivers Licence.
