The Team & Role
At Home Subacute is a dynamic, multidisciplinary service delivering high‑quality, evidence‑based care to patients in their own homes—care that would otherwise be provided in a hospital setting. Operating seven days a week, Peninsula At Home brings together allied health, nursing, pharmacy and medical professionals who are committed to keeping people well, supported and independent in their community.
As part of the At Home Subacute team within the Peninsula Care Group, you will work with patients requiring rehabilitation, geriatric evaluation and management, and palliative care. Care is delivered flexibly and creatively, using a mix of home visits, clinic appointments, group programs and telehealth to meet individual needs. The team values collaboration, clinical excellence and person‑centred care, with a strong focus on timely, coordinated discharge planning.
This casual Grade 2 Social Work role provides comprehensive psychosocial assessment, intervention and case management to clients within the At Home Subacute and Hospital in the Home (HITH) programs. You will work closely with patients, families and carers, and partner with internal and community services to support safe, effective transitions of care. The role places a strong emphasis on evidence‑based practice, clear goal setting, risk identification and collaborative decision‑making within a supportive multidisciplinary environment.
Based across the Peninsula Care Group catchment, this role offers variety, autonomy and flexibility, with opportunities to work across different sites and care settings while being well supported through clinical and organisational supervision.

Skills & Experience Required
- Member or eligibility of the Australian Association of Social Work.
- Current Victorian Driver’s license.
- Highly developed interpersonal and communication skills.
- Demonstrated ability and experience to work within a multidisciplinary team.
- Ability to independently deliver high quality social work services, including comprehensive assessment, counselling, patient management, and thorough discharge planning
- Previous Social Work experience in a public health setting and good working knowledge of community support services.
- Demonstrated awareness about home based services.
- Demonstrated knowledge and application of Social Work theory.
- Ability to manage a demanding workload and effectively prioritise tasks
