What You Will Be Doing  
At Home Subacute has a focus on shifting rehabilitation care out of the hospital setting, and into the home environment, to empower people to optimise their recovery, function and quality of life, with the support and guidance of home based rehabilitation experts.

The Grade 2 Speech Pathologist works within the multidisciplinary Peninsula At Home team, providing evidenced based, person centred care to patients with a broad range of communication, cognitive and swallowing disorders, to achieve optimal outcomes for clients. This may include a combination of centre based, home and community based and telehealth service delivery.

What You Need 
We anticipate you will have: 
•    Eligible for practising membership of Speech Pathology Australia.  
•    Demonstrated excellence in Adult Speech Pathology service provision in accordance with person-centred care principles. 
•    Ability to independently perform high quality speech pathology services in the community setting, including assessment, treatment, patient management, and thorough discharge planning. This included the neurological caseload. 
•    Demonstrate ability and experience working within a multidisciplinary team
•    Ability to work autonomously in the community setting and be self-directed.
•    Ability to adapt approach to meet both the needs of the consumer and the service.
•    Current and valid Victorian Driver’s licence.

*Please refer to Allied Health Professionals (Victorian Public Sector) (Single Interest Employers) Enterprise Agreement 2021-2026 salary circular for salary range. Sub-grading will be based on years of experience.
