What You Will Be Doing
The Transition Care Program (TCP) is a jointly funded program (DHS/Commonwealth) and it aims to assist frail aged patients reach their maximum functioning capacity and to help determine any supports or services which would be of benefit in supporting clients to remain independently at home.
The successful applicant will be working in the community providing support to TCP patients in their own homes across the Peninsula Health catchment area. The Speech Pathologist will need to demonstrate sound clinical judgement, assessment and practice skills, and have an understanding of the issues affecting older patients with complex clinical and psychosocial needs.

What You Need
We anticipate you will have:
- Eligibility/Membership of relevant professional body
- Experience in a sub-acute and/or community setting
- Demonstrated experience with supervising students
- Knowledge of / experience working with older people.
- Current Driver's Licence
- Interest and/or experience in Quality improvement and research activities
- Computer literacy

*Please refer to Allied Health Professionals (Victorian Public Sector) (Single Interest Employers) Enterprise Agreement 2021-2026 for salary range. Sub-grading will be based on years of experience.
