Job Description: 
  • Permanent Part Time - 72 hours per fortnight
  • Located in Frankston and Mornington Peninsula
  • Classification: Social Worker Grade 2 Year 1 - Year 4 (SC21 - SC24) or Community Development Worker Class 2B Year 1 - Year 7 (XX7 - ON15)* plus super
  • 5 weeks Annual Leave
  • Hybrid Working Options Available

The Team & Role 

 

Join Bayside Health’s Support at Home team, supporting older adults to live independently with choice and dignity. As a Care Partner, you’ll coordinate personalised care, complete assessments, develop care plans, connect clients with services, and support them to manage their care budgets.

Working in a collaborative team, you’ll advocate for clients, monitor wellbeing, and deliver responsive, person-centred care aligned with aged care standards while contributing to continuous improvement and positive outcomes.

 Skills & Experience Required 

  

  • Tertiary qualification in a relevant field such as: Social Work, Allied Health, Community Services
  • Current registration with relevant professional body (e.g., AHPRA for allied health professionals, AASW for social workers)
  • Valid Victorian Driver’s Licence
  • National Police Check and Working with Children Check (or willingness to obtain)
  • Demonstrated experience in care coordination, case management, or service navigation within aged care, community health, or disability sectors
  • Proven ability to develop and monitor consumer-directed care plans and manage service budgets
  • Well-developed consultation, problem solving, negotiation, coordination and facilitation skills
  • Exceptional time management skills and ability to work autonomously in the community
