Skills & Experience Required
- Allied Health Professional; tertiary qualification in Social Work or relevant certificate relating to community development, ENs can apply under the Comm Dev Award
- Demonstrated experience in case management or care coordination and delivering health outcomes for disadvantaged clients
- Experience undertaking comprehensive psychosocial assessments or client goal setting and planning
- An applied understanding of the impact of homelessness for clients with complex health and social needs
- Experience working in the community and a strong commitment to Social Model of Health and Trauma Informed Practice principles
- The ability to work autonomously and unsupervised
- Sound IT knowledge and skills.
- A valid Victorian drivers licence and willingness to transport clients as necessary
