The Team & Role 

 
The SRS Clinician works within the Supported Residential Services Outreach and Assistance Program (SRS OAP), supporting residents across six local Supported Residential Services. The role provides outreach, clinical support and care coordination to residents with complex health, social and behavioural needs. Working closely with the SRS Partnership Manager and outreach services, the clinician helps residents access health and community supports, strengthen wellbeing, and maintain safe, stable accommodation.

Skills & Experience Required 

 

  • Allied Health Professional; tertiary qualification in Social Work or relevant certificate relating to community development, ENs can apply under the Comm Dev Award
  • Demonstrated experience in case management or care coordination and delivering health outcomes for disadvantaged clients
  • Experience undertaking comprehensive psychosocial assessments or client goal setting and planning
  • An applied understanding of the impact of homelessness for clients with complex health and social needs
  • Experience working in the community and a strong commitment to Social Model of Health and Trauma Informed Practice principles
  • The ability to work autonomously and unsupervised
  • Sound IT knowledge and skills.
  • A valid Victorian drivers licence and willingness to transport clients as necessary
