Bayside Health was formed following the merger of Alfred Health, Bass Coast Health, Gippsland Southern Health Service, Kooweerup Regional Health Service and Peninsula Health on 1 January 2026.
The Team & Role
The Early Intervention in Chronic Disease (EIiCD) Team is a multidisciplinary Community Health team at Bayside Health Peninsula, delivering holistic, client‑centred care to support people living with chronic and complex conditions. The team works collaboratively to empower clients to achieve their goals, optimise independence and promote long‑term self‑management.
As a Grade 2 Physiotherapist within EIiCD, you will deliver predominantly centre‑based physiotherapy services to clients with musculoskeletal, respiratory, cardiovascular, diabetic and neurological conditions. You will complete comprehensive assessments and develop individualised, patient‑centred care plans, delivering interventions through face‑to‑face appointments, telehealth, phone consultations and group programs. Working closely with colleagues across disciplines, the role contributes to high‑quality, evidence‑based care, clinical supervision, and continuous quality improvement within the physiotherapy service.

Skills & Experience Required
- Current registration with AHPRA.
- Experience working in an outpatient setting.
- Proven ability to work effectively within multidisciplinary teams.
- Demonstrated commitment to delivering high quality outcomes for consumers.
- Demonstrated clinical knowledge and skills commensurate with experience.
