The Team & Role

 

The role of the HBCC Patient Liaison Nurse is to lead, coordinate, and deliver (as required) the continuity of the Safe, Personal, and Effective, Connected care for cancer patients in their home environment.

Hospital in the Home is a seven days a week service that maintains the continuum of acute inpatient care. This care can be provided in the patient’s home, residential care facility or other community setting.

Skills & Experience Required

  • Registered nurse with current AHPRA registration
  • Experience in working in cancer services and in administration of anti-cancer treatments (SACT)
  • Current Victorian drivers licence
  • Current IV Cannulation Accreditation/CVAD competency
  • The Staff Immunisation Questionnaire must be completed by all new employees as part of the mandatory on-boarding process. New employees are not permitted to undertake any activity that requires patient contact prior to clearance by the Staff Immunisation Nurses; this also includes evidence of having the Flu vaccination on a yearly basis. New employees with contra-indications to vaccinations or who decline vaccination are required to attend a face-to-face consultation with the Immunisation Nurses for appropriate documentation of their contraindications or declination. This must be completed prior to commencing any work with Bayside Health.

 

Desirable 

  • Demonstrates excellent communication skills
  • Understanding of the Home Based Cancer Care and Hospital In the Home policies/procedures
  • Excellent computer skills in common applications i.e. Microsoft Office programs
