On 1 January 2026 five health services (Kooweerup Regional Health Service, Peninsula Health, Alfred Health, Bass Coast Health and Gippsland Southern Health Service) will come together to create Bayside Health. A newly merged health service with more than 22,000 employees caring for a community of over 1.2 million Victorians.
The new health service will provide people with the best possible care with services that support them throughout their lives. The merger will also strengthen our workforce, providing new training, education and career opportunities. Subject to final approval on 1 January 2026, all employees from the five health services will transfer to Bayside Health.
The Team & Role
Peninsula Health’s Cognitive, Dementia and Memory Service (CDAMS) is a specialist diagnostic service for people with declining memory and changes to their thinking (cognition), servicing clients, and supporting their carers, in the Frankston and Mornington Peninsula areas. CDAMS provides assessment, diagnosis, education and support, referral and links to other services as required. CDAMS is a short-term service that does not provide case management or long-term, ongoing support.
As a Cognition Clinician you will be an integral part of the CDAMS team providing evidence informed care to clients who attend the clinic, through advanced clinical skills, effective communication and active participation within a multidisciplinary team environment. The position involves completing comprehensive assessments, risk assessments where necessary, participate in the operations of delivering a safe, timely and equitable service to clients.
As a Cognition Clinician you will have relevant experience in assessment of people with a cognitive impairment, interest in best practice for people with cognitive impairment, and commitment to delivering a safe, effective, person centred care to achieve optimal health outcomes to the clients and their family/carers.

Skills and Experience Required
- Current registration with National Registration board (AHPRA) for registered professions or eligible for membership of relevant professional association for non-registered professions
- Demonstrated experience in assessment of people with a cognitive impairment
- Demonstrated knowledge of best practice of the diagnostic process for people with a cognitive impairment
- Demonstrated knowledge of aged care, community supports and services
- Excellent interpersonal and written communication skills
- Demonstrated organisational ability in time management
- Demonstrated ability to work autonomously, flexibly and conjointly with staff from other disciplines as part of a client focused multidisciplinary team
- Understanding and commitment to relevant policies and procedures
- Demonstrated commitment to professional development, through maintaining a current knowledge of relevant literature and clinical development
- Victorian Drivers Licence
