Bayside Health was formed following the merger of Alfred Health, Bass Coast Health, Gippsland Southern Health Service, Kooweerup Regional Health Service and Peninsula Health on 1 January 2026.

 

The Team & Role

The Social Support Program sits under the Early Intervention In Chronic Disease Program area, which supports the health needs of adults 65 years and older, younger adults with a disability and adults with a chronic condition and/or psycho social issues. 
The Social Support Program runs social support groups from Hastings and Mornington Community Health. One group is an active group of younger (under 60 years) clients with a disability such as an acquired brain injury/intellectual disability and the other groups are older adults with various physical disabilities, dementia, social isolation and carer support needs. 
The program aims to maintain skills, provide meaningful activities including outings that enhance wellness and reablement while linking clients and carers to health services as required. 
The Social Support Group Worker aims to assist participants of the program to engage in social activities whilst endeavouring to maintain skill retention and development. The program is staffed by a Team Leader and social support group workers with additional help from invaluable community volunteers. The position provides both clinical & non-clinical duties as prescribed by the Team Leader to ensure consumers receive high quality, timely, safe and effective care.

Skills & Experience Required

Certificate III in Aged Care and Disability or Certificate III in Lifestyle and Leisure
Experience in working in a Social Support group program
Committed to delivering high quality care for clients
Ability to cooperate and contribute to team goals
Computer skills and ability to utilise common Microsoft applications
Competent communication skills both verbal and written
Communicates well with, relates to and sees issues from the perspective of people from a diverse range of cultures and backgrounds
Current Driver’s License
Ability to drive 12 seater bus – no special licence required
