Bayside Health was formed following the merger of Alfred Health, Bass Coast Health, Gippsland Southern Health Service, Kooweerup Regional Health Service and Peninsula Health on 1 January 2026.

 

The Team & Role 

At Home Subacute has a focus on shifting rehabilitation care out of the hospital setting and into the home environment, to empower people to optimise their recovery, function and quality of life with the support and guidance of home-based rehabilitation experts. The service operates 7 days a week and comprises allied health, nursing, pharmacy and medical staff, delivering care under six streams including Geriatric Evaluation and Management, Rehabilitation, Palliative Care, Reconditioning, Orthopaedic and neurological. This Grade 2 Physiotherapy role, as part of the At Home Subacute team, delivers evidence-based physiotherapy assessment and intervention to patients requiring rehabilitation, geriatric evaluation and management and palliative care in the home and community. Care is delivered flexibly and includes a combination of home visits, clinic visits, group therapy and telehealth consultations, as well as physiotherapy services to the Hospital in the Home (HITH) program, inclusive of Home Based Cancer Care, as required. The position is responsible for ensuring best practice standards in physiotherapy and rehabilitation across a wide variety of conditions and will deliver services across the Peninsula Health catchment, requiring flexibility to work across sites.

 Skills & Experience Required 

  • Current registration with AHRPA
  • Able to competently perform high quality Physiotherapy assessment, treatment and client management, including discharge planning, across a variety of clinical areas
  • Demonstrated ability to work in multidisciplinary team environment including commitment to effective communication with consumers and team members

  • Ability to work autonomously in the community setting and be self-directed
  • Highly developed verbal and written communication skills

  • Current Victorian Drivers Licence
