Bayside Health was formed following the merger of Alfred Health, Bass Coast Health, Gippsland Southern Health Service, Kooweerup Regional Health Service and Peninsula Health on 1 January 2026.
The Team & Role
Peninsula At Home Subacute has a focus on shifting rehabilitation care out of the hospital setting and into the home environment, to empower people to optimise their recovery, function and quality of life, with the support and guidance of home based rehabilitation experts. At Home Subacute is a 7 day a week service comprising of allied health, nursing, pharmacy and medical staff. Care is delivered under 6 different streams, including Geriatric Evaluation and Management, Rehabilitation, Palliative Care, Reconditioning, Orthopaedic and neurological. This position also provides occupational therapy services to the Hospital In The Home (HITH) program, inclusive of Home Based Cancer Care (HBCC), as required.
This grade 2 Clinical or Health Psychology role will be required to provide evidence-based assessment, interventions (including brief interventions) and behavioural management, and assist with linking patients into appropriate and ongoing psychological support pathways post-discharge. Part of the role will also include supporting the wider medical, nursing and allied health teams in case formulation, patient management and discharge planning. Referrals will be varied due to the complexity of medical conditions and co-morbid mental health issues that may be contributing to challenges in recovery. There is a strong emphasis on evidence-based practice, client focused goal setting with clear timelines and a collaborative team discharge.
This position will be required to deliver services across the Peninsula Health catchment, and as such, may be required to work flexibly across any of the Peninsula Health sites.

Skills & Experience Required
• General registration as a Psychologist with the Psychology Board of Australia
• Current driver’s license
• Experience in provision of brief interventions
• Highly developed interpersonal and communication skills
• Ability to independently deliver high quality psychology services and mental health support, including comprehensive assessment, counselling, patient management and thorough discharge planning
• Awareness and compliance with Australian Psychological Society Code of Ethics
• Evidence of excellent time management, adaptability, and team work within multidisciplinary teams
• Ability to manage workload and effectively prioritise tasks
• Established computer literacy skills
Desirable
• Demonstrated knowledge and application of psychological support for patients engaged in rehabilitation
• Previous experience in working within community or sub-acute settings
• Previous experience in working within psycho-oncology and/or palliative settings
• Previous psychological experience in a public health setting and good working knowledge of community support services
• Demonstrated awareness about home based services
• Health or Clinical Psychologist endorsement
