The Team & Role 

 

Seniors Lifestyle Victoria is an innovative, Department of Health–funded program that supports older adults to improve their health, wellbeing and independence through self‑management and community connection. Working within a multidisciplinary team, the Coordinator partners closely with clients to develop personalised Action Plans aligned to their goals, using health coaching, motivational interviewing and evidence‑based interventions.

 

This role is the primary point of contact for clients over a short‑term engagement, delivering and coordinating outreach services across homes, community settings, Peninsula Care Group and external providers. The Coordinator plays a key role in integrated care planning, service coordination and data collection, while also contributing to the establishment and continuous improvement of this exciting pilot program.

Skills and Experience Required 

 

•    Eligibility / Membership of relevant professional body 
•    Current Victorian Driver’s licence  
•    Proven ability to work effectively within multidisciplinary teams  
•    Demonstrated commitment to delivering high quality outcomes for consumers 
•    Demonstrated clinical knowledge and skills commensurate with experience 
•    Motivational Interviewing or Health Coaching experience and understanding
