What You Will Be Doing
Peninsula Health is seeking a permanent & full time Grade 2 Social Worker to join our team at The Mornington Centre. In this role you will provide support to older adults who are receiving inpatient care in a GEM setting. You will be responsible for completing psychosocial assessments, providing emotional and adjustment support and provide targeted interventions with both individuals and their families. You will be required to have a sound knowledge of the psychosocial issues that older people in our local community experience & will have experience in navigating relevant service systems. You will have demonstrated experience in working collaboratively to achieve patient goals and develop patient-centered discharge plans. You will receive regular supervision and support from a dedicated onsite Senior Social Worker, and you will also provide supervision to Grade 1 Social Workers and students.

What You Need
We anticipate you will have:
∙ You will need to hold a Bachelor Degree or Qualifying Masters in Social Work and two years of relevant post qualification work experience. Post graduate studies are highly regarded and may be eligible for a higher education allowance.
∙ As a multi-site organisation you may be required to work across sites. You will have access to our extensive pool of fleet cars and will need a Victorian Drivers licence.

*Please refer to salary circular for Allied Health Professionals (Victorian Public Sector) (Single Interest Employers) Enterprise Agreement 2021 – 2026 for salary range. Sub-grading will be based on years of experience.
