On 1 January 2026 five health services (Kooweerup Regional Health Service, Peninsula Health, Alfred Health, Bass Coast Health and Gippsland Southern Health Service) will come together to create Bayside Health. A newly merged health service with more than 22,000 employees caring for a community of over 1.2 million Victorians.
The new health service will provide people with the best possible care with services that support them throughout their lives. The merger will also strengthen our workforce, providing new training, education and career opportunities. Subject to final approval on 1 January 2026, all employees from the five health services will transfer to Bayside Health.
The Team & Role
The Supported Residential Services Outreach and Assistance Program (SRS OAP) provides funding, care coordination and support to 6 local SRS (Supported Residential Services) to improve living standards and quality of life for residents. The SRS Clinician works with the broader Homelessness and Health Outreach Service (HHOS), which provides case management and assertive outreach to people who are homeless or at risk of homelessness. The service works from a Housing First model to find suitable, long-term housing, for clients. The Service works to maintain people in suitable housing by wrapping around health and wellbeing services and develop people’s social connections with their community to improve people’s health and wellbeing.

Skills and Experience Required
- Degree in social work and eligible for membership
- Demonstrated experience in case management or care coordination and delivering health outcomes for disadvantaged clients.
- An applied understanding of the impact of homelessness for clients with complex health and social needs.
- Experience working in the community and a strong commitment to Social Model of Health and Trauma Informed Practice principles.
- A current drivers licence and willingness to transport clients as necessary
