The Team & Role
Make a system wide impact where clinical expertise meets patient flow. Bayside Health is seeking an experienced and highly skilled Clinical Nurse Consultant to join our Medicine teams as a Specialty Medicine Care Integrator within the Peninsula Care Group.
This is a senior clinical leadership role for a nurse who thrives in complexity, values collaboration, and is passionate about improving patient outcomes through proactive care coordination, timely decision-making, and integrated discharge planning.
The Specialty Medicine Care Integrator works in close partnership with specialty medicine medical teams as a senior clinical leader and key interdisciplinary resource. The role provides advanced clinical oversight, operational coordination, and expert support for patients admitted under the Medicine cluster, with a strong focus on:
- Managing complex, high‑risk admissions
- Proactively identifying and resolving barriers to patient progression
- Improving patient flow, reducing length of stay, and supporting safe, timely discharge
This role acts as a central point of clinical integration—linking medical, nursing, allied health, operational teams, and external services to deliver coordinated, person‑centred care aligned with patient goals and system priorities. This is a permanent part time role working 64 hours per fortnight
Key responsibilities
- Provide advanced clinical assessment and leadership for complex and high‑risk medical patients
- Identify patients at risk of prolonged length of stay and intervene early
- Lead and coordinate proactive discharge planning for long‑stay patients
- Support medical teams to navigate care pathways, community services, and alternative models of care
- Actively participate in Rapid Rounds, MDT discussions, family meetings, and long‑stay reviews
- Drive flow improvement strategies, including pre‑10am discharges and substitute care models (e.g. @Home, subacute, TCP)
- Contribute to LOS reporting, data analysis, and continuous quality improvement initiatives
- Educate, mentor, and influence clinicians to strengthen shared accountability for patient progression

About you
You are a confident senior nurse who can influence performance through collaboration with consumers and across multidisciplinary teams supported by senior operational and clinical staff. You bring a strong patient‑centred lens, excellent communication skills, and a demonstrated ability to partner effectively with diverse stakeholders.
Skills & Experience Required
- Current registration as a Registered Nurse with AHPRA
- Advanced clinical expertise in medical nursing care
- Demonstrated experience coordinating complex care and resolving discharge barriers
- Strong critical thinking, leadership, and interpersonal skills
- Ability to work collaboratively across hospital and community settings
- Current Police Check and Working with Children Check
- Postgraduate qualifications in nursing or working towards - Highly desirable
- Experience in patient flow, discharge planning, or system integration roles
