🩺 Why You'll Love This Role

Imagine starting your day with a home visit, helping a patient achieve meaningful goals and build confidence in their recovery journey. As part of our At Home Subacute service, you'll work across a diverse caseload spanning rehabilitation, geriatric evaluation and management, and palliative care, delivering care through home visits, clinics, group programs and telehealth. You'll collaborate closely with a skilled multidisciplinary team, provide clinical leadership and supervision, contribute to innovative service improvements, and play an important role in delivering high-quality, person-centred care where it can make the greatest impact, in the patient's own home. No two days are the same, offering variety, flexibility and the opportunity to make a genuine difference in people's lives.

The Team & Role

Join our At Home Subacute service, a dynamic seven-day-a-week program dedicated to helping people optimise their recovery, function and quality of life within the comfort of their own home. Working as part of a collaborative multidisciplinary team of allied health, nursing, pharmacy and medical professionals, you'll play a key role in delivering person-centred care across rehabilitation, geriatric evaluation and management, and palliative care streams.

As a Senior Speech Pathologist, you will provide high-quality, evidence-based care to patients across the home and community setting. Through a flexible service model incorporating home visits, clinic appointments, group therapy and telehealth consultations, you'll support consumers to achieve meaningful outcomes, maximise independence and enhance wellbeing.

This is a rewarding opportunity to apply your advanced clinical expertise across a diverse caseload while contributing to best-practice service delivery. Working closely with the Allied Health Lead, you'll support optimal care pathways, provide clinical supervision and mentoring, contribute to workforce planning, and help drive continuous improvement initiatives across the service.

If you're passionate about innovative models of care, enjoy working collaboratively, and are looking for an opportunity where your leadership and expertise can make a genuine difference, we'd love to hear from you.🌟
