• Classification Range: Social Worker Grade 2 Year 1 - 4 (SC21 - SC24)* or Community Development Worker Class 2B Year 1 - 7 (XX7 - ON15)*

• Salary: $1570.00 - $1955.00 (per full time week)*

What You Will Be Doing 

Join our team as an SRS Clinician, where you'll play a vital role in enhancing the wellbeing of residents in local Supported Residential Services (SRS). You will conduct comprehensive, client-centered assessments to identify care goals, address unmet health and social needs, and manage complex care situations to prevent instability and evictions. Collaborating with the Homelessness and Health Outreach Service (HHOS), you'll help connect residents with long-term housing solutions and provide essential support services to improve health outcomes and foster community connections. This position is based at Mornington Community Health centre, but may require work across all Peninsula Health sites. 

 

Key Responsibilities

  • Recognise, respond and escalate changes in the patient’s condition
  • Develop expertise to support residents and SRS staff manage behaviours of concern and complex co-morbidities
  • Develop client centred Care Plans coordinating services and engaging programs that build on residents’ strengths, empower residents, and build resident capacity to self-manage and self-advocate
  • Expand residents’ capacity to access mainstream health, welfare and community services, including linking into NDIS and My Aged Care Services

 

What You Need

 We anticipate you will have:

  • Allied Health Professional; social worker or community development worker
  • Demonstrated experience in case management, care coordination or public health programs
  • An applied understanding of the impact of homelessness for clients with complex health and social needs
  • Dynamic clinician with excellent time management skills and highly developed communication and documentation skills
  • A strong commitment to Social Model of Health and trauma informed practice principles
  • A valid driver's licence and willingness to transport clients as necessary

*Please refer to the salary circular for Allied Health Professionals Enterprise Agreement (Vic Public Health) for salary range. Sub-grading will be based on years of experience.
