On 1 January 2026 five health services (Kooweerup Regional Health Service, Peninsula Health, Alfred Health, Bass Coast Health and Gippsland Southern Health Service) will come together to create Bayside Health. A newly merged health service with more than 22,000 employees caring for a community of over 1.2 million Victorians.
The new health service will provide people with the best possible care with services that support them throughout their lives. The merger will also strengthen our workforce, providing new training, education and career opportunities. Subject to final approval on 1 January 2026, all employees from the five health services will transfer to Bayside Health.
The Team & Role
The Team Leader for the Aged Care Assessment Service (ACAS) is responsible for providing day to day leadership, supervision and operational coordination of the service. The role ensures high quality, timely and personal centred assessments in line with the Aged Care Act 2024, Support at Home Reforms, and Peninsula Health’s policies and procedures. The role ensures the delivery of client-centred, comprehensive, fair, and timely assessments that empower older Australians and individuals with disabilities to access Commonwealth-funded aged care services, both in the community and inpatient hospital settings.The Team Leader works closely with the Program Manager in providing leadership, practice development, and operational guidance to the aged care assessment team. This position is critical in supporting high-quality service delivery, enhancing consumer experiences, and fostering partnerships with key stakeholders. Additionally, the Team Leader collaborates with internal and external stakeholders by building positive relationships with service providers, hospital social workers, GP’s and community agencies to ensure optimal outcomes for all clients.

Skills and Experience Required
• Tertiary qualifications in allied health with current AHPRA registration or eligibility for membership with relevant professional body
• At least 10 years’ post graduate experience in the relevant profession desired
• Previous experience with an Aged Care Assessment Service highly regarded.
Comprehensive understanding of aged care reforms and changes to aged care legislation and ability to implement this through collaboration with staff and stakeholders.
• Experience leading teams in community service settings and working with diverse client groups, including aging populations, individuals experiencing mental illness, homelessness, or disability, and their carers/families.
• Demonstrated understanding of community-based support services and service system.
• Familiarity with culturally safe and inclusive practices for working with indigenous and non-English speaking backgrounds.
• Developing leadership skills with the ability to build cohesive teams and manage competing priorities.
• Strong organizational, strategic planning, and analytical skills, with proficiency in data collection, report writing, and computer applications.
• Excellent written and verbal communication skills, and the ability to foster community networks and partnerships.
• Current Class C driver's license

*Please refer to Allied Health Professionals (Victorian Public Sector) (Single Interest Employers) Enterprise Agreement 2021-2026 salary circular for salary range. Sub-grading will be based on years of experience.
